Patient's last name

Enter the patient's last name as shown on the invoice.

On clicking Proceed you will go to the eWAY payment gateway where you enter your card details and confirm payment.

Please note that a 1.32% credit card transaction fee plus 20 cents surcharge will be automatically added to your account.
